We have provided a quick list of items for you to provide that will expedite your evaluation and treatment process!

1. LIST OF ANTIBIOTICS & OTHER MEDICATION FOR SINUS YOU’VE TAKEN IN THE PAST YEAR
Usually your pharmacy will be able to provide you with your medication records.

2. LIST OF OTHER ROUTINE MEDICATIONS (HIGH BP, CHOLESTEROL, ETC.)
Usually your pharmacy will be able to provide you with medication your records. Certain medications have side effects that can lead to sinus problems.

3. MEDICAL DISEASES
We would like to review a list of your current medical diseases to better create a treatment plan that will work for you.

4. ANY SURGICAL SINUS PROCEDURES
Please provide us with a list of any surgical sinus procedures you’ve had in the past to better create a treatment plan for you.

5. HAVE YOU HAD A CT SCAN?
If you have already had a CT scan, please bring the disc. This will expedite your treatment process.

6. ALLERGY TESTING (ENVIRONMENTAL & FOOD)? PLEASE BRING RESULTS.
Please bring the results of any allergy tests you may have taken. This will expedite your treatment process.

7. IMMUNE SYSTEM BLOOD-WORK
If your doctor has ran blood-work on your immune system, please bring the results. This will expedite your treatment process.
